ATTENTION SELLERS:
All Livestock Producers please stop by the office to sign or print and return a Combined Continuous Affidavit (COOL Affidavits and Drug Residue Free) form below and return it to the office as soon as possible. If you have previously signed a Cool Affidavit we ask that all slaughter cattle and fat cattle sellers resign the form and return it to the office! Thank you!
